Registration

To register for Summer Arts at the Cabaret, please review the following information. If you have any trouble downloading the forms below, please contact us and we’ll send you a registration packet immediately.

We are filling up very quickly and spaces are limited so please send in your registration form and deposit just as soon as you can.

MEDICAL FORM

If you filled out a medical form last year and it was within three years of your last physical, you do not need to send in another health form. New medical forms are due prior to the first day of the program. PAs will not be allowed to start the program without this form. You will find the medical form on the last page of this packet.

PROGRAM TUITION
A $100.00 non-refundable deposit is required for all Performing Artists (PAs) to secure a place in the program. A place will be held for your child once the registration form and deposit have been received. We take program participants on a first come first serve basis. Full tuition is $1,250.00, and tuition balance is due on or before May 15th, 2010.
